How to Request a Transcript

  • Download, complete and print the SAHS Transcript Request Form (PDF)*
    *Please note: This transcript request form is for SAHS graduates only; current SAHS students must contact the guidance office directly to have transcripts sent to colleges or universities to which they are applying.
  • Be sure to include the following information on the form:
    • Your full name at graduation (maiden if applicable)
    • Telephone number
    • Date of birth
    • Year of graduation or last year attended
    • Number of transcripts needed
    • Address of school or organization where transcript is to be mailed

Please note the following guidelines for submitting transcript requests:

  • The Guidance Office requires 7-10 business days from when a request is received
  • Payment by cash or check payable to SAHS (cash preferred) $2.00 per transcript
  • Transcripts without payment will not be processed
  • Requests must be submitted by mail or walk-in
    *Requests will not be accepted via fax or e-mail

Completed transcript requests can be mailed or dropped off, along with payment, at:
